Homenetmen Liability Waiver

ABAGA SOUTHBAY POLICIES

REGISTRATION DETAILS

I understand that registering online does not guarantee enrollment in the requested class. Placement and age requirements must be met. Abaga staff will inform me if a class change needs to be made.
Abaga cannot guarantee activity or teacher requests and teacher’s schedules are subject to change at any time.
If Abaga contacts me regarding my child’s attendance and I fail to respond, my enrollment may be cancelled.
All children must be accompanied by an adult at all times while in class or partaking in Abaga activities including field trips.
Abaga does not offer credits or refunds for missed lessons.

PAYMENT DETAILS

Tuition is collected in one lump sum for the entire calendar year.
Tuition will be pro-rated for students starting later in the year.
All payments should be made to Homenetmen
Abaga agrees to give a 30-day notice in the event of a tuition rate change.

CANCELLATION DETAILS

I understand that I may cancel enrollment at any time. By cancelling, my child(ren) will be removed from the Abaga program.
All cancelations must be written and can be submitted via: https://www.abaganorcal.org/contact-us
If I cancel in the middle of the year, I understand that the yearly tuition fee is non-refundable.
All cancellations are final. To re-enroll re-register is required; in the event of a waiting list, I will be placed on the list along with other new registrations.

ATTENDANCE DETAILS

I will notify Abaga staff of planned absences to allow the program coordinators to plan accordingly.
I, or a responsible adult, will attend all classes with my child(ren).
If Abaga contacts me regarding my child(ren)’s attendance and I fail to respond, my enrollment may be cancelled, Abaga will provide a written cancelation notice to the e-mail provided at registration.
I will update Abaga of any changes in contact information or enrollment status in a timely manner.

USE OF PHOTOS

I understand that my child’s photo may be used on social media or on websites to promote Homenetmen only.
